Gift Certificate Redemption Form

• To redeem a Gift Certificate online please:

  -Fill out the following form as completely as possible.  All fields marked with a "*" are required. 

  -Once the form has been received we will process the order and Email a receipt or contact you if the sale amount is greater than the amount on the Gift Certificate. Credit Cards or checks may be used for purchases over the amount on the Gift Certificate.

  -Please keep in mind that a sales tax of 9.5% will be added to all sales shipped within Illinois.

  -Orders will be shipped once full payment has been received if pieces are immediately available.

  -All ground shipping within the USA is free until further notice. 

  -Any unused amount the of the Gift Certificate will stay on the Gift Certificate which has no expiration date.

  -Gift Certificates can be redeemed for Jewelry from Michele A. Friedman only.  Gift Certificates have no cash value.

  Gift Certificates may also be redeemed in person at any retail show.  See schedule here.
